McLeod Home Health provides quality health care for patients outside a hospital setting who require extended care after being released from the hospital. McLeod Home Health has assembled a team of skilled health care professionals, such as nurses, physical and occupational therapists, and social workers, to address each patient's individualized care plan.
Nursing & Physical Therapy Services
McLeod Home Health offers a range of services: including skilled nursing and physical therapy; to individuals who are no longer inpatients but require medical care for a limited period. Our Triad Team Care Model uses an RN Case Manager, RN Staff Nurse and an LPN Staff Nurse on each patient's case to ensure patient satisfaction. Patients meeting Medicare guidelines may qualify for third-party payment for Home Health Services.
